ISO 14001 environmental nonconformity report template
What is recorded when something fails in the environmental system or an incident affects the surroundings: how it was contained, what impact it had and what changes as a result.
In one sentence
The environmental nonconformity report records the breach or incident, the measures to control it and mitigate its environmental impacts, the cause analysis, the corrective action and its effectiveness, because ISO 14001 requires evidence to be retained of the nature of each nonconformity, the actions taken and their results (10.2).
- Where the standard requires it
- ISO 14001 · Clause 10.2 Clauses are cited with the ISO 14001:2015 numbering, the same one the Academy uses. The 2026 edition was published on 15 April 2026 and 2015 certificates must transition to it before 30 April 2029.
- Type of information
- Mandatory record Documented information to be retained (10.2)
- Who fills it in
- Whoever detects it and the process owner The environmental manager validates the analysis and verifies closure
- When
- As soon as it is detected Effectiveness is verified later, allowing enough time to see whether it recurs
- How long it is kept
- Set by the organisation The standard sets no periods: it asks you to decide on retention and disposition (7.5.3.2). Covering at least the three-year certification cycle lets you show the history at recertification.
What the standard requires
What ISO 14001 demands and what it leaves to your judgement
The cycle is the same as in ISO 9001 —react, analyse the cause, act, verify effectiveness—, with one difference worth reflecting in the template: the environmental standard explicitly asks you to deal with the consequences, including mitigating adverse environmental impacts.
Actions must be appropriate to the significance of the effects, including the environmental impacts. And where necessary the system has to change, which in practice usually means reviewing the aspects matrix, an operational control or the emergency plan.
A legal breach found during the compliance evaluation, a justified neighbour complaint or an actual spill are typical nonconformities for this record.
Structure
Fields the template should include
- Number, date, location and who detected it
- Origin: incident, legal evaluation, complaint, audit, monitoring
- Description and requirement not met
- Actual or potential environmental impact
- Containment and mitigation of the impact
- Notification to the authorities, if applicable
- Cause analysis
- Corrective action, owner and deadline
- Effectiveness check
- Changes to aspects, controls or emergency plan
Example
Example: washing water discharged to the surface water drain (fictitious company)
Company, people and figures are fictitious: the example only shows what a properly completed record looks like.
| Field | Content |
|---|---|
| Number and date | NC-AMB-2026-04 · 2 June 2026 |
| Origin | Incident spotted by the cleaning operator |
| Description | Mould washing water with detergent poured into the surface water drain instead of the process network |
| Impact | About 300 litres towards the municipal storm sewer; no visible effect on the watercourse |
| Mitigation | Surface water valve closed and the retained water pumped into a container for the waste manager |
| Notification | Town council notified the same day, as required by the discharge permit |
| Cause | The temporary washing area had no connection to the process network and no signage |
| Corrective action | Washing only in the connected area and drains signposted · production manager · 20 June |
| Effectiveness | 30 September: no recurrence · effective |
| Changes | New aspect «accidental discharge of washing water» added to the matrix as an emergency |
At the audit
Mistakes auditors commonly flag
- The corrective action is recorded but not what was done to mitigate the impact.
- Environmental incidents handled only as workplace accidents.
- No assessment of whether the authorities had to be notified.
- The aspects matrix is not updated after a new type of incident.
- Effectiveness is signed off the same day.
In Kimobox
How this record is kept in Kimobox
- 1Each environmental nonconformity is opened with mandatory root cause.
- 2The corrective action is assigned an owner and a date, with a warning if it slips.
- 3The effectiveness of the action is verified and the aspects matrix updated if needed.
- 4Repeated nonconformities feed the system’s risk map.
Frequently asked questions
What people ask us before getting started
Is it the same form as for ISO 9001?
It can be in an integrated system, but with two fields the quality standard does not explicitly require: the environmental impact and its mitigation. Without them, the record does not show that the environmental consequences were dealt with.
Is an incident with no damage still a nonconformity?
If it breaches a requirement —an operational control, a permit condition—, yes. The absence of damage reduces its severity, not the obligation to analyse the cause.
