SQAS templates and records for the assessment
Nine records a chemical haulier needs to answer the SQAS questionnaire with evidence, separating what the questionnaire itself asks for from what the ADR already requires.
In one sentence
An SQAS assessment does not lead to a certificate but to a report in which an independent assessor answers each question with yes, no or not applicable on the strength of the evidence seen; that is why the records —driver training, behaviour observations, incidents, emergencies, subcontractors, maintenance, security of the load, the safety adviser’s report and the improvement plan— decide the score more than the procedures do, and many of them are mandatory under the ADR as well.

SQAS2022 templates and records
SQAS does not number its requirements the way a standard does: its Core questionnaire is split into three areas —management; safety, health, environment, quality, security and social responsibility; and contracting of services— and is complemented by the specific questionnaire of each module, such as Transport Service. Each template states the area and, where the obligation is a legal one, the relevant ADR chapter.

Frequently asked questions
What people ask us before getting started
Does SQAS require specific records?
The questionnaire imposes no forms, but the assessor can only score a 1 on seeing objective evidence, and for questions flagged as requiring a comment must note which evidence was seen. In practice, no record means no point.
How does it relate to the ADR?
SQAS assesses safety and quality management beyond the law, yet many questions rest on ADR obligations: driver training, the safety adviser, instructions in writing or the security plan. If a legal record is missing, the matching question does not score either.
Who sees the report?
For the first month, only the assessed company, which may add its comments. After that it is published in the SQAS database for participating chemical companies, except those the company chooses to exclude.
