ISO 45001 templates and records

Nine records from an occupational health and safety system, each with the clause that calls for it, the Spanish legal duty that usually comes with it, its fields and a completed example.

In one sentence

ISO 45001 requires risks and opportunities and emergency plans to be maintained as documented information, and evidence to be retained of competence, performance monitoring, the evaluation of legal compliance and every incident with its actions; in Spain those records sit alongside the ones imposed by Law 31/1995 —risk assessment, prevention planning, health checks and the list of accidents—, and a well-designed system handles each with a single record instead of keeping two.

Frequently asked questions

What people ask us before getting started

Which records are mandatory under ISO 45001?

Those the standard says must be “retained”: evidence of competence (7.2), monitoring and measurement results (9.1.1), results of the compliance evaluation (9.1.2), the audit programme and audit results (9.2.2), management review results (9.3), and incidents and nonconformities with their actions (10.2). In addition, risks and opportunities (6.1.1), legal requirements (6.1.3) and the emergency process and plans (8.2) must be maintained as documented information.

Can the prevention service’s documentation be used for ISO 45001?

Yes, to a large extent. The risk assessment and prevention planning required by Spanish Law 31/1995 are the foundation of clause 6.1.2. What the standard adds is that those documents belong to a system: they are reviewed when things change, they feed objectives and their effectiveness is checked.

Do incidents without lost time have to be recorded?

ISO 45001 defines an incident broadly, including events that cause no harm, and requires them to be investigated and evidence retained. In Spain, accidents without sick leave are also reported in a monthly list through Delt@.
